Dua tahun setelah kejadian Final Fantasy VII, penyakit yang disebut Geostigma menyebar di seluruh planet. Penyakit ini dipercaya disebabkan oleh material asing yang menyerang tubuh, akibat kejadian di Final Fantasy VII dua tahun lalu. Merasa bersalah dan dihantui masa lalunya, ex-SOLDIER Cloud Strife memilih hidup sendiri, jauh dari teman-temannya sambil bekerja di "Strife Delivery Service", yang bermarkas di lokasi bar Tifa Lockheart, Seven Heaven. Seven Heaven juga merawat anak-anak yang terkena Geostigma. Disini, Tifa tinggal bersama Marlene anak angkat Barret dan juga Denzel (salah satu anak yang terkena Geostigma), sedangkan Barret mencari energi alternatif untuk menggantikan energi Mako. Suatu hari, Cloud mendapat panggilan telepon dari Shira Company, presiden Shinra Company ingin agar Cloud melindunginya dari tiga pria misterius, salah satu diantaranya bernama Kadaj, mereka sedang mencari "Ibu", dan mereka percaya kalau Cloud mengetahui dimana mereka dapat menemukanya. Siapa sebenarnya ketiga orang tersebut? Apa yang dimaksud dengan "Ibu"? Apa ada hubunganya dengan kebangkitan Sephiroth?

Two years after the events in "Final Fantasy VII", a disease called 'Seikon-Shoukougun', or 'Geostigma', is spreading through the planet.
